Wavier Guidelines
- I, the athlete/parent/guardian, understand that participation in sports, including volleyball, carries inherent risks, and it is impossible to guarantee the safety of all participants. Volleyball requires cardiovascular fitness, coordination, and agility. While efforts will be made to minimize risks, I acknowledge that not all risks can be avoided. Therefore, I release Moxie Volleyball Lab, its employees, and educators from any responsibility for any injury that may be sustained by my child during regular lessons.
- I further understand that in the event of a medical emergency, Moxie Volleyball Lab will promptly call EMS for assistance, and I accept financial responsibility for any related expenses.
- During volleyball activities, accidents can occur, ranging from minor collisions with other players or the ground, to more significant injuries such as sprains, fractures, concussions, and heat stroke. To reduce the risk of injury, all participants are expected to be physically fit and follow the rules of volleyball, thereby creating a safe playing environment. I agree to abide by the safety rules provided by Moxie Volleyball Lab, report any unsafe practices or conditions, and promptly inform the educator of any health-related conditions that may affect my child’s ability to participate.
- Telephone, Email, and SMS Text Messaging Terms and Conditions: By clicking “I accept,” I consent to receive email, text, and phone communications from Moxie Volleyball Lab.
- I certify that the above-mentioned participant possesses the necessary physical fitness to engage in volleyball safely. I understand that my child must discontinue activity if they experience undue discomfort or stress, and I have disclosed any relevant health-related conditions in the “Special Needs” section. Furthermore, I will inform the Moxie Volleyball Lab educator immediately of any changes or concerns. Having read and understood the information provided, I am aware of the risks associated with participating in volleyball, and I willingly allow the above-mentioned participant to take part in the activity. By doing so, I acknowledge that they assume all inherent risks associated with the sport. I also understand that in case of a medical emergency, Moxie Volleyball Lab will seek appropriate medical assistance, and I will be responsible for any resulting expenses.
- I hereby grant Moxie Volleyball Lab permission to capture photographs and videos of my child during Moxie Volleyball Lab lessons. These visual materials may be used on the company website (www.moxievolleyballlab.us), company social media pages, and promotional materials without revealing my child’s name or identity. If I do not wish for photographs or videos of my child to be taken, I will notify Moxie Volleyball Lab via email.
